Clients of RideConnect of FSW must be physically and cognitively capable of traveling independently. If the client does not meet these requirements, they must be accompanied by an aide or companion for all rides.
In addition, clients may choose to have an aide or companion accompany them on rides. We welcome passengers ages eighteen and over.
If a client will be accompanied by an aide or companion, they MUST notify RideConnect of FSW when submitting their ride request.
If the client neglects to inform RideConnect of FSW of an aide or companion at the time the ride request is made, and an aide or companion is present on the day and time of pickup, the volunteer or bus driver may choose to decline the trip.
This Acknowledgement and Waiver form must signed by the aide/companion before accompanying the RideConnect of FSW client being transported by a RideConnect of FSW volunteer driver or bus driver.
